I have a complaint. How can I contact the Ombudsman's Office?
It's
easy. Just access Anvisatende and fill out the form, send a fax or call
the Ombudsman's Office through phone number (61) 448-1235, 448-1464
or through phone/fax 448-1144.
Please note that the actions of the Sanitary Surveillance - VISA are
decentralized; therefore the municipalities and states are responsible
for carrying out inspection of premises subject to sanitary surveillance.
Those organs must be set in motion every time appropriate measures need
to be adopted to address irregular facts observed in your town.

Is identification compulsory?
No.
It's optional. However, if you choose to remain anonymous, the Ombudsman's
Office will not be able to inform you of the result of your complaint
or denunciation. In case you wish to identify yourself and want your
personal information to be stored, the Ombudsman's Office undertakes
to keep it confidential.

Is there a model for the mail?
No.
You can write any way you want, but make sure that the information is
precise and thorough, to ensure that the issue is quickly addressed
through the appropriate channels.

How long will it take to get an answer?
We try to act as quickly as possible, although in some cases more time
is needed, due to forwarding of the issue to other areas and/or institutions.
Until the citizen gets an answer, the Ombudsman's Office never drops
the case..

Does the Ombudsman's Office solve any issue?
It
must be made clear that the main function of the Ombudsman's Office
is to intermediate the demands of users/citizens upon the organs that
make up the National Sanitary Surveillance System, ensuring the defense
of citizens' rights.
6. And the denunciations?
In
cases that involve finding and investigation, it is not enough to call
and denounce. The Ombudsman's Office needs a complete report on the
matter, documented proof, as well as everything that may be used to
help reach a solution. Citizens should make an official written denunciation
and provide all personal information. Denouncing over the telephone
is not the most appropriate.
